The solution, named FrailCare, aims to provide a holistic and personalised platform for frailty detection and self-management that will utilise non-intrusive wearable and IoT technology to collect real-time physiological and activity data. FrailCare will use interoperable data formats to allow the integration of external data sources and leverage state-of-the-art AI to provide frailty risk assessment and prediction, adverse event detection, care pathway matching and decision support. Moreover, FrailCare has a scope to facilitate self-management through frailty-specific Shared Care Plans and provide physiological and cognitive rehabilitation exercises through virtual and augmented reality serious games.

Finally, it will promote the engagement of older people in real-world social activities through a social community platform and will support the frailty-related education and emotional support of the older persons through intelligent virtual assistants.
